REVIVE SAUNA SUITES LLC
SERVICE AND LIABILITY WAIVER
Thank you for choosing Revive Sauna Suites for your Infrared Sauna, Red Light Therapy, and wellness needs. Before booking your appointment, please carefully read and agree to the following Service and Liability Waiver:
Health Considerations
By checking the box, you acknowledge and agree:
1. You understand that Infrared Sauna Therapy, Infrared Light Therapy, Red Light Therapy, Halotherapy, and all other services offered are not a substitute for professional medical advice, diagnosis, or treatment.
2. You are encouraged to consult a licensed healthcare professional before using these services, especially if you have a pre-existing medical condition. If you have concerns about medications or potential contraindications, you agree to consult with your physician or pharmacist before participation or use.
3. You accept full responsibility for consulting your medical professional. If advised not to use these services or devices, you agree to refrain from participation or use of them. You are also responsible for informing Revive Sauna Suites LLC of any recommended temperature or time restrictions from your healthcare provider.
4. If you choose to participate without medical clearance or against medical advice, you do so at your own risk and accept full responsibility for any outcomes.
Medical Contraindications
By checking the box, you acknowledge and agree that:
Infrared Sauna Therapy is contraindicated for:
- Individuals with a fever
- Heat-sensitive individuals
- Those with cardiovascular conditions, including hypertension
- Individuals with pacemakers
- Pregnant individuals (consult with a physician)
- Children under 12 (must have medical clearance)
- Elderly individuals (consult a physician; lower temperatures and shorter sessions are recommended)
Other conditions to consider:
- Chronic conditions affecting the ability to sweat (e.g., Anhidrosis, MS, Diabetes with neuropathy, some autoimmune conditions)
- Medications that may impair the body's heat response — consult a pharmacist or physician
- Hemophilia or bleeding disorders
- Recent joint injury (within 48 hours)
Red Light Therapy (LED Light Therapy) is contraindicated for:
- Individuals with light sensitivity
- Epilepsy
- Those using photo-sensitizing medications
- Individuals prone to migraines triggered by light
Halotherapy (Salt Therapy) is contraindicated for:
- Fever or infectious diseases
- Respiratory cancers
- Tuberculosis
- Cardiac diseases
- Acute respiratory illnesses
- Severe hypertension
- Chronic Obstructive Pulmonary Disease (COPD)
- Pregnant individuals (consult your physician)
Vibration Plate Use
Not recommended for individuals with:
- Cardiovascular conditions
- Muscle or bone injuries
- Medical implants
- Epilepsy
- Pregnancy (consult your doctor)
- Children under a certain age
Use caution if you are:
- Elderly
- Overweight or obese
- Experiencing back pain or recent injuries
Conduct and Safety Agreement
By checking the box, you agree to:
- Conduct yourself respectfully during your visit
- Follow all instructions for safe and proper use of equipment
- Discontinue use if you experience dizziness, light-headedness, or signs of heat exhaustion
- Be at least 18 years of age (those under 18 must have written parental consent)
- Immediately stop use if you feel pain or discomfort
- Wear protective eyewear during Red Light Therapy (provided or bring your own)
- Avoid alcohol consumption before sauna use, as it increases heart rate and may compound heat stress
Liability Release
By checking the box, you acknowledge and agree that:
- Revive Sauna Suites LLC, its employees, and affiliates are not liable for any injuries, discomfort, or adverse reactions resulting from participation or use of its services, products, or devices.
- You voluntarily assume all risks associated with participation and use of the Infrared Sauna, Red Light Therapy, Halotherapy, Vibration Plate, and other wellness services or products provided by Revive Sauna Suites LLC.
- You, your heirs, executors, representatives, and assigns release Revive Sauna Suites LLC and its owners, officers, and employees from any liability or claims arising from your participation.
- This waiver remains in effect for all current and future use of Revive Sauna Suites services and devices, unless revoked in writing.
- Revive Sauna Suites is not responsible for lost or stolen items or for damage to personal electronic devices used inside the sauna.
Appointment Cancellation and Rescheduling Policy
By checking the box, you agree:
- A minimum of 2 hours’ notice is required for cancellations and 1 hour for rescheduling.
- If an appointment is canceled more than 2 hours before the appointment time, a refund will be issued minus credit card processing fees.
- No-shows will not receive a refund and the session will be marked as used (if part of a plan or package).
- No-shows without notification may result in prepaid-only booking privileges.
By checking the box, you confirm that you have read, understood, and agree to abide by the terms outlined in this Service and Liability Waiver.
